Patina Health Champion

Imani “Mani” Wilson is a Patina Health Champion who is passionate about helping older adults live well and thrive. Mani specializes in care coordination and navigation, making sure her patients’ goals and preferences stay front and center while advocating for their care.

As a Health Champion, this includes helping patients overcome barriers to care, connecting them to community resources, answering questions about benefits and eligibility, arranging things like rides to appointments and support for personal care needs, coordinating with other providers and specialists, and more. She is especially proud to support Patina’s efforts to bridge health disparities among older adults in underserved communities — a role she embraces with genuine warmth and dedication.

Mani is currently pursuing a Bachelor of Science in psychology, with a minor in neuroscience, at Pennsylvania State University. She also holds phlebotomy and EKG certifications from the New York Medical Career Training Center. Before joining Patina, Mani spent seven years as an aftercare clinical assistant at CityMD, working closely with physicians and care teams to help patients smoothly transition from urgent care to follow-up appointments. Earlier in her career, she served as a member engagement specialist at Healthmap Solutions, providing resources to support kidney disease patients.

Residing in Charlotte, NC, Mani enjoys spending time with her family. In her free time, you might spot them exploring the city’s beautiful parks, trails, and nature preserves or enjoying a bowl of ramen at Jinya Ramen Bar – one of their favorite local spots! Mani also loves expressing herself through makeup and beauty, bringing creativity and color into everything she does. Above all, Mani’s dedication to advocating for patients’ well-being is at the heart of her work, making her a true asset to the Patina team.
