Patina focuses exclusively on caring for older adults and augments the care you already provide, extending your reach and ability to support patients’ total health and well-being over time. Our multi-disciplinary team brings expertise in aging and geriatric care, and includes physicians, nurse practitioners, behavioral health specialists, health advocates and other specialized resources. We directly engage patients to understand their health goals, reinforce care plans, close care gaps, coordinate care with other providers and specialists, proactively check in, and deliver in-home care when needed – and coordinate with you to close the loop.

Depending on a patient’s health insurance plan, we can provide care and support in two different ways, delivering an added level of support to the care you already provide through our Patina Total Health service, or if you prefer, serving as a patient’s primary care provider if needed.

Simply refer a patient to us and we’ll take it from there, making sure there's a smooth transition and prompt access to care, if needed.*
