With Patina primary care, you’ll have a dedicated team, including a board-certified primary care practitioner (PCP), registered nurse and health champion. Your PCP will work with you and others on your team to develop a personalized care plan so you can achieve your health and wellness goals.

We’ll also bring in other Patina experts in areas like behavioral health, pharmacy, condition management, healthy aging, and more when needed. If you’d like any loved ones to be part of your care, we make it easy for them to join appointments and stay updated on our app.

Where are you located?

We’re physically located right outside Philadelphia, but we’ll be coming right to you virtually and in-person. You’ll never need to come to the office! We currently support patients who live in the following counties:

Pennsylvania – Bucks, Chester, Delaware, Montgomery, Philadelphia
North Carolina – Cabarrus, Gaston, Iredell, Lincoln, Mecklenburg, Rowan, Union
South Carolina – Chester, Lancaster, York

How do you provide high-quality care, virtually?

We are a virtual-first practice, which means most of your interactions with us will be via phone, video or our web and mobile apps. This makes it easy for us stay in touch whenever you need – wherever you are. The app is simple to use and was built with our patients in mind. In just a few clicks, you can send a message, request a prescription refill, join a video visit and more.

When eyes-on, hands-on care is needed, we’ll come to you. At-home visits include all the same things you can typically do in a doctor’s office and we bring everything that’s needed with us to your home.

Is there a limit on the number of visits or how often I talk to you?

There is no limit to the number of visits you have in-home or virtually. And, we’re always just a phone call or click away on our mobile and web apps if you’re not feeling well or simply have questions.

Additionally, we tailor your care to your unique goals and needs. As your goals and needs change, so does your care plan. That’s the beauty of having a dedicated team who stays with you over time.

What does a health champion do?

Your dedicated health champion will take on whatever is needed to help you navigate your care. That includes assisting in making appointments, arranging transportation, coordinating lab work and prescription refills, explaining insurance benefits and more. Whether our patients are relatively fit and active, or managing one or more conditions, they love having their health champion by their side.
