Why Patina Total Health matters
Today’s healthcare system is fragmented, complex, and overwhelming — especially for older adults. It often overlooks the unique needs that come with aging, including changes in function, mobility, and having to manage one or more complex / chronic conditions.
Older adults deserve better. Patina Total Health simplifies healthcare and makes it age-friendly by providing a trusted care team that stays with patients every step of their healthcare journey. Our care team — made up of physicians, nurse practitioners, registered nurses, behavioral health specialists, and health champions, all of whom are also experts in aging — becomes a single point of contact for whole-person care.
