Q&A: Chris Connell
At Patina, relationships are at the heart of everything we do – and Chris Connell embodies that mission every day. As a Health Champion, Chris plays a vital role in delivering personalized, compassionate care to older adults, helping them navigate their health with confidence.
In this Q&A, Chris shares what makes his role unique, what he values most about working with patients, and why he believes listening is one of the most powerful tools in healthcare.

Q: Tell us a bit about yourself.
I’m a Health Champion at Patina, where I support older adults in navigating their healthcare journey and making informed decisions about their well-being. I’m passionate about building genuine relationships with my patients – taking the time to understand who they are, what matters to them, and how I can best support their health goals. What I love most about my role is being part of a team that prioritizes whole-person care and focuses on the full picture of someone’s life – not just a list of symptoms. It’s all about helping our patients live their best lives.
Outside of work, I enjoy spending time with my family, trying out new recipes in the kitchen, and rollerskating. It keeps me active and brings me joy!
Q: What were you doing before you joined Patina?
Before Patina, I worked in outpatient physical therapy. That experience really shaped my approach to care, with a focus on helping people meet their health and wellness goals. One of the most rewarding parts of that role was watching patients gain strength and confidence over time. It gave me a strong foundation in listening closely to what each person needed – physically and emotionally – which is something I carry with me into every interaction I have at Patina.
Q: What drew you to Patina?
What immediately stood out to me about Patina was its patient-centered philosophy. In traditional healthcare settings, patients can feel like just another name in a chart. At Patina, care is built around the individual – their lifestyle, their goals, their voice – and not just their medical needs. The idea of being a consistent, dedicated partner for patients, not just someone they hear from when it’s time for an appointment, really resonated with me. I wanted to be part of a practice that prioritizes trust, respect and meaningful connection. And that’s exactly what I found here at Patina.
Q: What are some of the things you can do as a health champion that the average primary care office wouldn't be able to do?
As a Patina Health Champion, I am able to provide a level of personal support and continuity that’s rare in most traditional care settings. I’m not here to just schedule appointments – I am helping to guide patients through every step of their care journey. That might mean helping them understand a new diagnosis, walking them through insurance questions, locating community resources, or simply being someone they can call when they don’t know where to start. We build relationships over time, so our patients know they are never facing things alone. That kind of trust and partnership doesn’t exist in most healthcare settings.
Q: How do you work with your patients at Patina?
I am focused on listening, understanding, and staying present. I get to know my patients on a personal level – what they care about, what challenges they’re facing, and what kind of support will they need. I check in regularly, help coordinate their care with others on their Patina care team or other providers, and make sure they feel informed and empowered to make decisions. Sometimes I’m helping patients set up a virtual visit or navigate a referral; other times, I’m simply offering a reassuring voice when things feel overwhelming. My goal is to make healthcare feel less complicated and more human.
Q: How do you work with patients’ family members who want to stay in the loop and be involved with their loved ones' care?
Family members are a vital part of the care team, and I do my best to ensure they feel informed, supported and heard – and always in a way that aligns with the patient’s preferences. Whether it’s providing updates, answering questions, or helping to bridge communication between loved ones and the clinical team, I’m here to make sure everyone is on the same page. I’ve dealt with times when a daughter is feeling anxious about her mom’s new medication, or a spouse isn’t sure how to manage caregiving responsibilities. Being able to step in with the information, encouragement and practical help they need can make a big difference for everyone involved.
Q: What life lessons have you learned from your patients?
One of the biggest lessons I’ve learned is the value of slowing down and truly listening. Many of my patients have lived incredible, full lives and they have so much wisdom and perspective to share. When I take the time to hear their stories – not just listen to their symptoms – it allows me to take a step back and figure out a more holistic care plan. I’ve also learned that even small gestures of kindness can have a big impact. A thoughtful follow-up call, reassuring word, or just being fully present in a conversation can go a long way in making my patients feel valued and supported.
Q: What is most rewarding about working at Patina?
Some of the most rewarding moments happen during my initial calls with new patients. I have had people tell me that they’ve not felt seen or heard in previous healthcare settings – that they’re usually rushed, dismissed, or treated like a number. When they realize that I’m here to truly listen and that they have a whole dedicated team behind them, there’s often a deep sense of relief and gratitude. Those conversations remind me why I do this work. Knowing that I am part of something that is improving healthcare outcomes for older adults is incredibly rewarding.
