As a Patina patient, you’ll have your own dedicated team, who can address any issue affecting your total health and well-being: physical, emotional, social or logistical care needs. Your team, experts in aging, will get to know you and stay with you over time, always listening and prioritizing your goals, values and preferences.

As a member of your dedicated care team, your health champion will coordinate with your Patina PCP and other providers, and help you access specialized resources as needed.
